About The Position

The Physical Therapist PRN plays a critical role in delivering high-quality rehabilitative care to patients on an as-needed basis across various healthcare settings in the United States. This position focuses on assessing, planning, and implementing individualized treatment programs to improve patients' mobility, reduce pain, and restore function following injury, surgery, or illness. The therapist collaborates closely with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care and optimal recovery outcomes. Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance

Nice To Haves

• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.
