About The Position

A Skilled Nursing Facility in Orange, NJ is seeking a licensed Physical Therapist (PT) to provide rehabilitative services to short-term rehabilitation and long-term care residents. This is a full-time, permanent role requiring prior experience in a Skilled Nursing or post-acute setting. The Physical Therapist will work collaboratively within an interdisciplinary team to support functional improvement, safe discharge planning, and regulatory-compliant documentation.

Requirements

Active New Jersey Physical Therapist license required
Minimum 1+ year of experience in a Skilled Nursing Facility or post-acute rehab setting required
Strong understanding of skilled documentation standards and Medicare guidelines
Ability to manage a productive caseload in a SNF environment
Strong communication and interdisciplinary collaboration skills

Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans
Provide therapy services to short-term rehab and long-term care residents
Monitor patient progress and modify treatment plans as appropriate
Maintain accurate, compliant documentation in accordance with Medicare and facility standards
Participate in interdisciplinary care plan meetings and rounds
Support discharge planning and functional goal achievement
Ensure compliance with Medicare Part A, Part B, and payer documentation requirements
