About The Position

FOX Rehabilitation is seeking passionate, licensed Physical Therapists to provide exceptional one-on-one care to older adults in Highland Heights and Westlake, OH. This role involves delivering care directly in patients' homes or within Senior Living Communities, utilizing the signature Geriatric House Calls™ model. Therapists will help patients improve mobility, independence, and quality of life. The position offers autonomy and flexibility, allowing clinicians to create and manage their own treatment schedules and build strong patient relationships. FOX clinicians also have the option to incorporate telehealth and virtual care, enhancing flexibility and continuity of care. This high-impact role is suitable for PTs who want to positively impact the lives of the communities served, working within a clinician-operated practice rooted in meaningful relationships, evidence-based care, and a mission to abolish ageism. FOX provides evidence-based rehabilitation services in various settings and fosters a supportive culture that values clinician-driven decision-making and professional fulfillment.

Requirements

  • Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification
  • Ability to meet the physical demands of in-home and community-based care, including: Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
  • Performing CPR
  • Providing moderate to maximum assistance with patient transfers and bed mobility
  • Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
  • Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
  • Wearing PPE when required

Responsibilities

  • Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
  • Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
  • Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
  • Document effectively within FOX’s EMR system in accordance with regulatory standards
  • Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
  • Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
  • Provide telehealth / virtual care when appropriate to extend access and flexibility
