Requirements
- Doctor of Physical Therapy (DPT) or equivalent (MPT, MScPT)
- Current and unrestricted Virginia Physical Therapy license
- Excellent communication and critical thinking skills
- Valid driver's license, active auto insurance, and CPR/BLS certification
Nice To Haves
- Two or more years of experience in home health, acute care, or rehab preferred
Responsibilities
- Deliver skilled, compassionate in-home therapy as part of a collaborative care team
- Evaluate and develop individualized treatment plans for adult and geriatric patients
- Serve as a key liaison between patients, families, and interdisciplinary team members
- Document care clearly and thoroughly in our EMR system
- Help shape the future of in-home rehabilitation
