Physical Therapist
About The Position
Arbor Post Acute is seeking a dedicated and compassionate Physical Therapist to join our skilled nursing and rehabilitation team. If you’re passionate about helping patients regain strength, mobility, and independence in a supportive, collaborative environment, we’d love to meet you. As a Physical Therapist, you will evaluate and treat patients following physician referrals, develop individualized treatment plans, and collaborate closely with an interdisciplinary care team. You’ll play a key role in patient recovery, staff education, and discharge planning while maintaining accurate and timely clinical documentation.
Requirements
- Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
- Licensed Physical Therapist in good standing
- Experience in post-acute or skilled nursing settings preferred
- Strong communication, teamwork, and documentation skills
- Proficiency with rehab and EMR systems (Casamba, PointClickCare preferred)
Responsibilities
- Evaluate patients promptly and develop effective, physician-approved treatment plans
- Provide hands-on therapy services and monitor patient progress
- Supervise Physical Therapy Assistants, aides, and students in accordance with state practice acts
- Collaborate with physicians, nursing staff, families, and the rehab team
- Participate in care conferences, utilization reviews, and discharge planning
- Educate patients, families, and staff on therapeutic programs and safe mobility
- Maintain compliant documentation and recommend appropriate durable medical equipment
