About The Position

A Registered Physical Therapist (PT) administers physical therapy to patients in their place of residence and is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Director/Manager of Patient Services. DUTIES & RESPONSIBILITIES Understands and adheres to established policies and procedures. Provides physician and IDG prescribed physical therapy. Improves or minimizes residual physical disabilities of the patient. Performs home safety assessments. Participates with all other health care personnel in patient IDG care planning. Directs and supervises personnel as required. Performs all procedures as ordered by physician. Consults with physicians and IDG regarding change in treatment. Instructs caregiver in use of good body mechanics for turning and lifting patients. Instructs patients and family/significant others in home programs and activities of daily living. Participates in in-service programs and presents in-service programs as assigned. Participates in QAPI activities as assigned. Attends all patient care conferences as scheduled. Prepares clinical and progress notes. Trains patient/family in use of adaptive equipment. Helps develop the plan of care and revise as necessary. Consults with family in addition to Home Health and Hospice personnel. Requirements

  • Licensed to practice as physical therapist within the State, if applicable.
  • Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master’s program in physical therapy approved by the Commission on accreditation in Physical Therapy Education (“CAPTE”) or any successor organization of the CAPTE.
  • Two years of appropriate experience as a physical therapist.
  • Ability to exercise initiative and independent judgment.
  • Acceptance of philosophy and goals of home health and hospice.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Nice To Haves

  • Community/home health experience is preferred.
