Job Summary:

• Interpretation of referrals in acute care, swing bed, school system, and/or outpatient facilities.
• Initial examination, evaluation, objective problem identification, diagnosis, and prognosis.
• Development or modification of an individualized plan of care that is based on the initial examination and that includes anticipated goals and expected functional outcomes.
• Supervisory responsibility for up to four licensed physical therapist assistants
• Coverage of patient care in outpatient, swing bed, and school systems, as instructed by Director of Rehabilitation. Caseload may require treatments in more than one setting, based on caseload needs and staffing dynamics
• Timely review and completion of all supporting documentation
• Establishment of discharge plan summary of status
• Direct questions and concerns to director of rehab in a timely manner

Requirements

• Minimum of Bachelor's degree in Physical Therapy from an accredited program with current licensure in the State of Mississippi
• Good overall mental and physical health
• Stamina and endurance for expected shifts
• Enhanced tolerate to continuous walking
• Ability to perform proper lifting > 60 pounds Occasional sitting
• Frequent lifting of 10-30 pounds

Responsibilities

• Creating thorough documentation for all patient populations
• Timely review of treatment documentation, re-examination of the patient and anticipated goals and expected outcomes, and revisions when indicated
• Establishment of discharge plans and appropriate discharge environment recommendations
• Supervision of assistants, up to four individuals at any given time
• Interpreting of physician referrals and obtainment of any required supporting patient documentation, including diagnostics, labs, appointment notes, etc., in order to assure comprehensive review and care of patients
• Assuring timely and appropriate care based on patient presentation
• Assisting with marketing of services to the community
• The physical therapist will also perform any other duties assigned by the director of rehabilitation and hospital administration.
• Providing prompt notice of absence or need for change of schedule with Director of Rehab via telephone
