Requirements

California-licensed Physical Therapist (PT)
CPR certification (AHA-approved)
At least 1 year of clinical experience (Home Health experience preferred)
Organized, compassionate, and reliable
Valid driver’s license, auto insurance , and reliable transportation
Strong communication skills and clinical decision-making

Nice To Haves

Home Health experience preferred

Responsibilities

Conduct in-home PT evaluations and follow-up visits
Design and implement individualized rehabilitation and treatment plans
Improve patients’ strength, balance, mobility, and functional independence
Educate patients and caregivers on safe movement and equipment use
Document visits using WellSky Kinnser EMR
Coordinate care with physicians, RNs, case managers, and the interdisciplinary team
