About The Position

Padua Community Services, an intermediate Care Facility (ICF), is seeking a licensed, part-time Physical Therapist to provide assessments and physical therapy services to individuals with intellectual and developmental disabilities in a resident care setting. The Physical Therapist will assess, plan, implement with physician orders, individualized treatment plans.

Requirements

• Doctor of Phyical Therpay (DPT) degree from an accredited physical therapy program.
• Or Bachelor's or Master degree in physical therapy.
• Current, unrestricted Physical Therapist license in the State of Louisiana.
• Experience providing services to individuals with physical and developmental disabilities.
• Ability to work independently while collaborating with an interdisciplinary team.
• Strong communication and documentation skills.

Responsibilities

• Conduct comprehensive physical therapy evaluations and reassessments in compliance with ICF requirements.
• Develop, implement, and update individualized physical therapy treatment plans based on physician orders.
• Provide direct physical therapy services to improve mobility, range of motion, posture and balance.
• Participate in interdisciplinary team meeting.
• Train and educate direct care professionals on therapeutic exercises, positioning, transfers, and mobility support.
• Monitor resident progress and document outcomes, making recommendations.
• Maintain accurate, timely clinical documentation in accordance with state ICF regulations and Padua policies.
• Collaborate with nursing, provide direct support of professionals and other staff to ensure coordinated care.
• All other duties as assigned
