About The Position

As a Physical Therapist, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function and mobility. You will assess patients' physical abilities and develop personalized treatment plans that incorporate evidence-based therapeutic techniques. Your work will involve collaborating with other healthcare professionals to ensure comprehensive care and optimal patient outcomes. You will also educate patients and their families on injury prevention, rehabilitation exercises, and lifestyle modifications to support long-term health. Ultimately, your efforts will contribute to enhancing patients' quality of life by restoring movement and reducing pain.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance

Nice To Haves

• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.
