About the Role: As a Consonus Physical Therapist, you’ll deliver person-centered, evidence-based care to highly motivated patients—many of them seniors—helping them regain function and independence. You’ll be part of a collaborative team, using advanced clinical pathways to guide therapy plans and elevate outcomes.
Requirements
- Graduate of an accredited program with a degree in Physical Therapy
- Current state licensure -or- in the process of obtaining appropriate state licensure
- CPR Certification (requirement varies based on State/location)
- National Provider Identifier (NPI) required upon hire
- Excellent organizational skills to facilitate program development
- Strives to positively represent their discipline and Consonus Rehab in the community

Responsibilities
- Design and implement personalized therapy programs using clinical data and Consonus’ proven pathways.
- Help patients maximize physical, emotional, social, and cognitive function.
- Work in a setting where patients are truly grateful, and your work makes a difference every day.
