Requirements

  • Current Wisconsin PT license
  • Strong clinical skills and a passion for working with older adults
  • Excellent communication and teamwork abilities
  • Flexibility, professionalism, and a positive attitude
  • Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
  • License as required by the State of Wisconsin to practice as a Physical Therapist.

Responsibilities

  • Provide compassionate and effective physical therapy services to adult and geriatric residents
  • Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
  • Support residents in achieving personal goals and rehabilitation milestones
  • Accurately document care and maintain compliance standards (experience with Net Health a plus)
  • Collaborate with therapy and nursing teams to ensure patient-centered outcomes
  • Educate residents and families on safe techniques and strategies to maximize independence
  • Participate in team meetings and contribute to care planning
