About The Position

Are you a Physical Therapist that has a desire to empower patients in their own home and guide them toward quality of life and overall safety? Are you ready to act as a patient advocate as your patient pursues their road to recovery? Connected Home Health is seeking experienced & caring Physical Therapists to serve patients on the East side of the greater Portland, Oregon community. We are looking for candidates that are experienced, patient-centered PT who is home health savvy, OASIS-C fluent, autonomous, team/culture oriented, driven, creative, fun (!), technologically adept, quality focused, positive, and overall awesome! Our Physical Therapists are supported by a trained Physical Therapy Assistant. Team meetings can be as easy as a phone call! About the Opportunity Provides physical therapy services to patients in their homes according to a written plan of care Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences Participates in discharge planning for patient Works closely with interdisciplinary team to ensure appropriate care coordination Prepares and submits a clinical progress or discharge summary based on the attainment of goals as directed by Connected Home Health policy Supervises Physical Therapy Assistants according to agency policy and state regulations

Requirements

• Degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization
• Licensed to practice as a physical therapist within the State of Oregon
• Two years of appropriate experience as a start of care physical therapist.
• Community/Home Health experience is preferred.
• Demonstrates good verbal and written communication and organization skills
• Current BLS for Healthcare Workers from the American Heart Association
• Must be a licensed driver with an automobile that is insured in accordance with state or Agency requirements and is in good working order

Responsibilities
