About The Position

The Center at CenterPlace is Greeley's premier, dedicated short-term rehabilitation facility featuring 54 private suites, state of the art rehabilitation equipment, and care delivered in a hotel-like setting. Our center is seeking a team-oriented Physical Therapist who is dedicated to patient outcomes and customer service. Essential Job Duties The Physical Therapist is responsible for the evaluation, plan of care, treatment, re-evaluation, discharge, and appropriate communications to patients and customers. Develop treatment plans, supervise assistants and aides, contribute to case management, and provide quality patient care. Maintain a positive level of interaction with patients, peers, and families. Enhance and expand client relations with facilities and their staff.

Requirements

• Must be a graduate of an accredited school for Physical Therapy
• National and state licensure REQUIRED to start
• Must have a Physical Therapist license
