Requirements
- Current Physical Therapist (PT) license in Pennsylvania.
- Graduate of an accredited Physical Therapy program.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Prior experience in home health or skilled nursing preferred—but we’re happy to train the right candidate!
Responsibilities
- Evaluate, plan, and implement treatment programs to help patients improve mobility, strength, and independence.
- Deliver care in multiple settings, ensuring a smooth transition and continuity of services.
- Collaborate with an interdisciplinary team to achieve the best outcomes for each patient.
- Educate patients and families on therapy techniques, safety, and home exercise programs.
- Document care in accordance with regulatory and company standards.
