About The Position

As a Physical Therapist specializing in home health care, you will play a critical role in improving the quality of life for patients recovering from injury, surgery, or managing chronic conditions within the comfort of their own homes. Requirements

• Successful completion of an accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance

Nice To Haves

• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis before intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Ensures continuity of quality patient care delivered with appropriate documentation.
• Provide effective communication to patient/family, team members, and other health care professionals.
• Promote agency philosophy and administrative policies.
• Maintain and complete all documentation within the company's stated guidelines.
