About The Position

Santa Rosa Post Acute is seeking a licensed Physical Therapist (PT) to join our dynamic and compassionate therapy team. We specialize in post-acute rehabilitation and skilled nursing care, helping our residents regain strength, mobility, and independence in a supportive, patient-centered environment. Requirements

  • Active California Physical Therapist (PT) license
  • Strong clinical decision-making and communication skills
  • Passion for working with geriatric and post-acute populations

Nice To Haves

  • SNF experience preferred, but new grads are welcome to apply

Responsibilities

  • Evaluate patients and develop individualized treatment plans
  • Deliver skilled physical therapy services to improve mobility, strength, and function
  • Collaborate with nursing and rehab teams to ensure optimal outcomes
  • Maintain accurate and timely documentation per SNF and regulatory standards
