The Physical Therapist is responsible for delivering direct care to patients and utilizing physical agents as prescribed by a referring physician.

  • Evaluates patient and creates treatment plan.
  • Employee is able to provide verbal and written educational material to patients when appropriate
  • Recognizes the need for involvement of other disciplines
  • Creates a recommended discharge plan
  • Performs and records daily clinical notes
  • Participates in performance improvement initiatives
  • Completes department mandatory in-service/training/competency within specified time frame
  • Participates in education of staff and students within the medical center
  • Bachelor of Science Degree in Physical Therapy or Master's degree or higher in Physical Therapy. Doctor of Physical Therapy (DPT) preferred.
  • Licensed and registered PT in the State of New York.
