About The Position

PACE Physical Therapists evaluate and implement individualized treatment plans to improve mobility, restore function, maintain safety, and reduce pain. The Physical Therapist will collaborate with the Interdisciplinary Team to provide all-inclusive care for our participants so they can age safely in their homes. Requirements

  • Physical Therapy degree (Bachelor, Master, or Doctoral).
  • Active State of Michigan licensure.
  • Must have one (1) year of experience with a frail or elderly population.
  • Physical ability to instruct and demonstrate exercise, provide lift assistance, and assist/manage mobility devices.

Responsibilities

  • Work in conjunction with the Interdisciplinary Team to coordinate rehabilitation services for participants at PACE Southeast Michigan.
  • Perform assessments, provide treatment sessions, implement care plans, and order/issue Durable Medical Equipment.
  • Document all care provided to participants in accordance with department standards and submit to medical records in a timely fashion.
  • Maintain accurate clinical records and utilization data relevant to the rehabilitation department.
  • Actively participate in professional associations, educational programs, and in-service education.
  • May delegate and direct the activities of support staff including Physical Therapist Assistant treatments & Rehabilitation Technician duties.
  • Complete initial and periodic assessments including home safety and facility assessments as needed.
  • Complete assessment documentation prior to and for engagement in participant care conferences.
  • Perform other related duties as determined by center clinical manager which may include site assignments, program development, and quality improvement projects.
  • Maintain awareness of participant safety issues within the Day Health Center and home, including mobility assist needs and DME.
