Requirements

  • Graduate of a university approved by American Physical Therapy Association
  • Licensed in the state of Iowa to practice physical therapy
  • Minimum of 1-year experience preferred

Nice To Haves

  • Community or home health experience desirable

Responsibilities

  • Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting.
  • Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.
  • Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.
  • Communicates with physician any requests to revise the plan of care.
  • Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.
