Greenfield Rehabilitation Agency is seeking a PRN Physical Therapist (PT) to join our growing therapy team at our senior living community in New Richmond, WI and its surrounding communities. Requirements
- Active Wisconsin Physical Therapist license
- Strong clinical, communication, and organizational skills
- Passion for working with older adults in long-term care settings
- Dependability, flexibility, and a team-player mindset
- Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
Responsibilities
- Evaluate and treat patients in a skilled nursing setting
- Develop individualized care plans to improve mobility, strength, and function
- Collaborate with the interdisciplinary team to ensure continuity of care
- Document patient progress in accordance with regulatory and company standards
