Requirements

  • Licensed Physical Therapist (PT) in the State of California
  • Strong clinical skills and dedication to resident-centered care
  • Ability to work independently as well as part of a team
  • Excellent communication and organizational skills

Nice To Haves

  • Skilled nursing or post-acute rehab experience preferred, but not required

Responsibilities

  • Evaluate residents to determine therapy needs and create individualized treatment plans
  • Provide direct physical therapy treatments in accordance with physician orders and resident goals
  • Document evaluations, progress notes, and treatment records accurately and timely
  • Collaborate with an interdisciplinary team to ensure comprehensive resident care
  • Supervise and support Physical Therapy Assistants (PTAs)
  • Participate in discharge planning and caregiver education to ensure continuity of care
  • Maintain compliance with state, federal, and facility regulations
