Requirements
- Licensed Physical Therapist (PT) in the State of California
- Strong clinical skills and dedication to resident-centered care
- Ability to work independently as well as part of a team
- Excellent communication and organizational skills
Nice To Haves
- Skilled nursing or post-acute rehab experience preferred, but not required
Responsibilities
- Evaluate residents to determine therapy needs and create individualized treatment plans
- Provide direct physical therapy treatments in accordance with physician orders and resident goals
- Document evaluations, progress notes, and treatment records accurately and timely
- Collaborate with an interdisciplinary team to ensure comprehensive resident care
- Supervise and support Physical Therapy Assistants (PTAs)
- Participate in discharge planning and caregiver education to ensure continuity of care
- Maintain compliance with state, federal, and facility regulations
