About The Position

As a Physical Therapist specializing in home health care, you will play a critical role in improving the quality of life for patients recovering from injury, surgery, or managing chronic conditions within the comfort of their own homes. Your primary objective will be to assess patients' physical capabilities, develop personalized rehabilitation plans, and implement therapeutic interventions that promote mobility, reduce pain, and prevent further disability. You will collaborate closely with patients, their families, and other healthcare professionals to ensure comprehensive care and optimal recovery outcomes. This role demands a compassionate approach, strong clinical expertise, and the ability to adapt treatment plans to diverse home environments. Ultimately, your work will empower patients to regain independence and enhance their overall well-being through targeted physical therapy services.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

Nice To Haves

• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.
