The physical therapist, whether contracted or employed through the Organization, is responsible for delivering physical therapy services in accordance with a physician's written plan of care and as defined by the state Physical Therapy Practice Act. This role involves assessing and evaluating patients' therapeutic, rehabilitative, and functional status, participating in the development of their total plan of care, and evaluating their home environment. The therapist will also assess muscle strength, mobility, gait, and range of motion to determine rehabilitation potential, direct physical therapy treatment, and instruct patients and caregivers on therapeutic appliances. Key aspects include identifying priority needs, reporting patient reactions or changes to the physician, and initiating physical therapy programs while instructing other personnel or family members on program goals. The therapist may also train patients in the use of prosthetic devices, identify needs for other home health services, and participate in discharge planning. Administrative duties include preparing and submitting clinical and progress summaries based on goal attainment and organizational policy. The role also involves providing physical therapy consultation, in-service education programs for nursing personnel, and participating in peer consultation. Supervision of physical therapy assistants and evaluation of home health aide care (when therapy is the sole skilled service) are also part of the responsibilities.
