HealthPro Heritage is seeking a Full-Time Physical Therapist to join their Home Health team in Casa Grande, Arizona. Requirements
- Degree in Physical Therapy from an accredited institution.
- Valid state licensure as a Physical Therapist, or license eligible.
- Strong clinical assessment and diagnostic skills.
- Proficient in developing and implementing effective treatment plans.
- Excellent communication and interpersonal skills with a patient-centered approach.
Responsibilities
- Provide physical therapy services to patients in their homes.
- Develop and implement personalized treatment plans.
- Collaborate with interdisciplinary care teams and other healthcare providers.
- Support patients in achieving functional independence and improved quality of life.
- Evaluate patients' physical conditions, including mobility, strength, and flexibility.
- Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain.
- Educate patients and families on exercises, posture, body mechanics, home routines, and lifestyle changes to aid recovery and prevent injuries.
- Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
- Participate in multidisciplinary team meetings as needed.
