About The Position

The physical therapist is responsible for proving physical therapy services and for adherence to all conditions outlined in the personnel services agreement.

Requirements

  • Bachelor’s Degree in Physical Therapy approved by an accredited organization.
  • PT License and CPR certification, if required by state
  • 2+ Years experience
  • Ability to show compassion and understanding.
  • Knowledge in HIPAA laws and confidentiality policies.
  • Ability to create and implement treatment plans based on patients’ specific condition and needs.
  • Ability to evaluate, conduct and development customized treatment plans based on findings.
  • Strong observation, time management, detail orientation, communication and interpersonal skills.
  • Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
  • Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
  • Ability to operate various equipment base on department.
  • The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms. Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.
  • For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
  • For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
  • For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Nice To Haves

  • Master’s Degree in Physical Therapy approved by an accredited organization.

Responsibilities

  • Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the development of the total plan of care. Evaluates home environment.
  • Assesses for muscle strength, mobility, gait, ROM – potential for rehab.
  • Directing physical therapy treatment.
  • Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
  • May train patient in the use of prosthetic device.
  • Participates in discharge planning for patient and in peer consultation process.
  • Provides physical therapy consultation to home families/caregivers when indicated and provides Inservice education programs for nursing organization personnel as needed.
  • When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.
