Requirements

Bachelor's or Master's degree in Physical Therapy from an accredited institution.
Valid Physical Therapist License issued in Michigan (MI).
At least one year of experience in a healthcare setting.
Proficiency in TruChart EHR or similar EMR systems.
Valid, regular driver's license.

Responsibilities

Collaboratively creating and implementing patient care plans with the home health team.
Ensuring optimal delivery of care to patients in Jackson and its suburbs through a flexible schedule.
Engaging in local travel to residential homes as part of the patient care delivery.
Adhering to critical charting deadlines and managing schedules efficiently.
