Requirements
- Must be a graduate of an accredited Physical Therapy Program
- Must have an active PT License in the State of California or be eligible to obtain one

Responsibilities
- Complete patient assessments to develop and implement an individualized care plan
- Provide high-quality, patient-centered care in collaboration with the patient, family, and care team
- Provide skilled, goal-oriented physical therapy while maintaining an ongoing assessment of the patient to make any changes to the care plan as necessary
- Maintain all records and reports per regulatory guidelines and coordinate with interdisciplinary teams
