This Physical Therapist (PT) role involves supervising, evaluating, and providing therapy services to clients within various senior living communities, including assisted living and independent living facilities. PTs provide 1:1 patient care, follow each patient from start to discharge, and collaborate daily with the facility's wellness team to ensure continuity of care. They examine and treat patients with physical impairments using physical modalities, aiming to improve mobility, function, independence in self-care, and other daily living skills to help patients age in place. Enhabit offers Specialty Programs such as Balance and Fall Prevention, Spine Safety, Low Vision, and Diabetic Management. Physical Therapists administer skilled care to clients requiring intermittent professional services and educate families and other healthcare team members, all in accordance with physician's orders and the established plan of care, under the direction of the Branch Director.

Requirements

  • Be currently licensed as a Physical Therapist (PT) in the state of employment
  • Demonstrate knowledge and skill in current therapy practice
  • Possess a valid state driver’s license and automobile liability insurance

  • A minimum of one year of clinical experience is preferred

Responsibilities

  • Supervise, evaluate, and provide therapy services to clients in various senior living communities, including assisted living and independent living facilities.
  • Provide therapy services to patients within assisted living facilities.
  • Provide 1:1 patient care.
  • Follow each patient from start to discharge.
  • Work collaboratively with each assisted living facility's wellness team on a daily basis to maintain strong lines of communication and overall continuity of care for the residents and their loved ones.
  • Examine and treat patients with physical impairments through the use of physical modalities.
  • Assist persons who are physically challenged to improve mobility and function, independence in self-care, and other skills necessary for functioning in daily living and to successfully age in place.
  • Administer skilled care to clients requiring intermittent professional services.
  • Teach the family and other members of the health care team.
  • Perform services in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the Branch Director.
