Physical Therapist - Skilled Nursing Facility Hours Available: 40 hours/week - 1-2 weekend shifts per month 24 hours/week - 1-2 weekend shifts per month Per Diem Mary Ann Morse Healthcare Center, a 118 bed skilled nursing facility located in Natick, MA, is looking for an Activity Assistant to join our team. Participates in the clinical team process. Provides teaching and training to patients, families and caregiver staff. Participates in clinical program development.

  • Screens patients according to policy.
  • Evaluates patients and develops appropriate plan of treatment.
  • Under a physicians order, provides evaluation and direct patient treatment in accordance with an established plan of care.
  • Contributes to the interdisciplinary team meeting/process.
  • Consults with and trains patients, families and caregivers.
  • Maintains productivity expectations on an ongoing basis.
  • Documents treatment according to company’s clinical documentation system on a timely basis.
  • Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.
  • Communicates effectively with patients, families, caregivers, team members and supervisor.
  • Promotes positive customer relations.
  • Participates in clinical program development and implantation.
  • Demonstrates ability for continuous new learning to remain current in the field.
  • Participates in quality improvement activities.
  • Exhibits courtesy, compassion and respect to patients, families, caregivers, visitors, physician, administrators and other team members.
  • Delivers services in compliance with the facilities ethical standards, policy and procedure, national professional standards, state practice acts where applicable, and current accepted state of the art practice.
  • Tasks and duties as assigned by the Rehabilitation Manger or Administrator as needed.
  • Bachelor of Science , Master of Science or Doctorate Degree in Physical Therapy and current professional licensure in the state of hire.
  • At least one year of work experience as a Physical Therapist working with adults.
  • Possess strong oral and written communication skills, Organizational and analytical skills required.
  • Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.
  • Demonstrated ability to utilize independent and effective clinical judgment, problem-solving and decision-making skills.
  • Demonstrates effective time management skills.
  • Orthopedic and manual skills are preferred.
