About The Position

We are a rural Critical Access Hospital committed to providing high-quality, personalized care to our community. Our Rehab services include inpatient, outpatient, and swing bed (subacute) rehabilitation, offering physical therapists the opportunity to make a meaningful impact across a variety of care settings. We are seeking a compassionate and flexible Physical Therapist (PT) to join our team on a PRN (as needed) basis. The PT will evaluate, plan, and implement individualized treatment programs for patients across multiple care settings, working closely with physicians, nurses, and other therapy professionals to promote recovery, independence, and wellness.

Requirements

  • Current licensure as a Physical Therapist (PT) in the state of Michigan.
  • Graduate of an accredited Physical Therapy program.
  • Strong communication, organization, and patient-centered care skills.
  • Ability to work independently and as part of a team.
  • Graduate from an accredited Physical Therapy Program
  • Current State of Michigan Physical Therapy License
  • CPR Certification
  • The candidate must be a team player and flexible with scheduling
  • Good communication skills required, both verbal and written form
  • Organizational skills with an attention to detail
  • Positive attitude and willing to help other departments

Nice To Haves

  • Prior experience in inpatient, outpatient, or swing bed settings preferred, but not required.
  • One year of experience in short term Rehab and/or Outpatient Therapy is desired.

Responsibilities

  • Provide physical therapy services for inpatient, outpatient, and swing bed (subacute) patients.
  • Complete assessments and develop individualized treatment plans.
  • Deliver evidence-based therapy interventions to improve mobility, strength, and function.
  • Educate patients and families on therapy goals, exercises, and discharge planning.
  • Collaborate with an interdisciplinary team to support patient progress and outcomes.
  • Document care accurately and timely in the medical record.
