About The Position

Tabitha's Home Health in Lincoln/Lancaster County, NE is seeking a Physical Therapist to work part time 8a- 4:30p. The Physical Therapist is responsible for providing Physical Therapy services to Tabitha’s Home Health Care Seniors/residents/clients with physical or cognitive disabilities to maximize functional independence.

Requirements

  • AGE REQUIREMENT: At least age 18.
  • EDUCATION: Bachelor's Degree in Physical Therapy required.
  • CREDENTIALS : Physical Therapist license in good standing as recorded on the Nebraska State Registry required.
  • CPR Required.
  • Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.
  • Must possess auto insurance at or above Nebraska statutory requirements.
  • All licenses, certifications, and permits must be kept up to date at the expense of the employee.
  • Must follow Tabitha’s vaccination requirements.

Nice To Haves

  • EXPERIENCE: One year experience as a Physical Therapist, preferred.
  • Geriatric experience, preferred.

Responsibilities

  • Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.
  • Performs Physical Therapy treatment procedures or delegates to a licensed Physical Therapist Assistant based on initial and on-going client evaluation.
  • Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.
  • Documents the care provided, instructions given, client's progress toward goals, and changes in condition adhering to Tabitha's policies and procedures and state/federal regulations.
  • Reports significant changes in the client's condition to the physician and nursing staff.
  • Communicates and coordinates care and client's progress with other team members involved in the care.
  • Designs, implements, and evaluates therapeutic exercise programs, and instructs the client's family/caregiver/Home Health Aide in carrying out the program.
  • Consults with the attending physician and obtains a written or telephone order for Physical Therapy plan of care and any changes in the client's plan of care as needed.
  • Ensures that assigned client’s clinical records are complete, timely, and reflect coordination of care.
  • Oversees and coordinates Physical Therapist Assistant and Rehabilitation Aide client care activities.
