About The Position

Join Transitions At Home, a not-for-profit home health organization, and help patients recover and thrive from the comfort of their homes. We’re hiring for a Part-Time (20–30 hrs/week) or Full-Time (30+ hrs/week) Physical Therapist to support our growing team in Southwest Wisconsin. Why Join Us? You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home. Requirements

• Active Wisconsin Physical Therapist license
• CPR certification
• Valid WI driver’s license & car insurance
• Two (2) years of experience preferred
• Comfortable working independently in a home care environment

Responsibilities

• Complete OASIS assessments, evaluations, and progress notes
• Develop and revise care plans
• Perform skilled physical therapy procedures
• Educate patients and families on home programs and daily activities
• Collaborate with physicians and the care team
• Participate in IDT meetings, in-service training, and QA initiatives
• Ensure accurate documentation and use of ICD-10 codes
