Job Summary: ● Ongoing interdisciplinary assessment of the patient. ● Develops, initiates, and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver (s). ● Provides skilled therapy services per physician orders, the patient’s plan of care, agency policies and professional standards. ● Performs all aspects of patient/client care in an environment that optimizes safety. ● Ensures that changes in patients’ clinical status are communicated to other healthcare team members and agency office staff in a timely manner. ● Obtains all necessary physician orders and collaborates with the physician as the patient's needs dictate. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. ● Coordinate patient care and participate in case conferences to discuss multidisciplinary team patient progress, plans for continued care, new problems, etc. ● Educates and counsels patients, their families/caregivers, and other staff as appropriate, in appropriate speech therapy modalities. ● Provides, modifies, or discontinues physical therapy modalities based upon an ongoing assessment/reassessment of the patient’s clinical status and in accordance with physician orders. ● Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs. ● Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age. ● Documents all care and services provided according to professional standards and agency policy and procedures. ● Instructs patients in proper in-home exercises and therapies. ● Educates patients and their families about the recovery process and the challenges they will face. ● Identify and meet patients’ goals and needs ● Provide quality, personalized and evidence-based care and proven interventions ● Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
