About The Position
The Physical Therapist is responsible for establishing, implementing, and evaluating a plan of care designed to restore function and prevent disability in patients following disease, injury, or loss of a body part.
Requirements
- Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience
- Illinois license in Physical Therapy
- BLS
Responsibilities
- Performs thorough evaluation
- Develops appropriate Plan of Care
- Implements Plan of Care
- Documentation is accurate, complete and timely
- Provides supervision to assistants, rehab techs, and students
