Requirements

  • Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
  • License as required by the State of Wisconsin to practice as a Physical Therapist.

Responsibilities

  • Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance
  • Identify barriers to functional independence and design individualized physical therapy programs
  • Address challenges related to mobility, posture, gait, and injury or illness recovery
  • Provide interventions to help residents regain strength, prevent falls, and maintain independence
  • Support residents in daily functional tasks, recommending adaptive equipment as needed
  • Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function
  • Assess living spaces to recommend modifications for safety and accessibility
  • Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention
  • Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care
  • Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies
