Job Purpose: Physical therapists contracted or employed through the organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the personnel service agreement.

Responsibilities

  • Provides physical therapy services to patients in accordance with the physician’s written plan of care. Services include assessment, treatment planning, and implementation of therapeutic interventions.
  • Assesses and evaluates patients’ functional status, including muscle strength, mobility, gait, range of motion, rehabilitation potential, and home environment, and determines physical therapy priorities.
  • Develops, initiates, and directs individualized physical therapy treatment programs to meet patient goals.
  • Instructs and educates patients, families, caregivers, and involved personnel on therapeutic exercises, treatment goals, safe techniques, and the use and care of therapeutic and prosthetic devices.
  • Monitors patient response to treatment and reports changes in condition or progress to the appropriate personnel.
  • Identifies patient and family/caregiver needs for additional home health services and makes referrals as appropriate.
  • Prepares and submits required clinical documentation, including evaluations, progress notes, and clinical summaries based on patient outcomes and attainment of goals, in accordance with organizational policy.
  • Participates in interdisciplinary case conferences and discharge planning.
  • Provides consultation and education to families, caregivers, and nursing personnel as indicated.
  • Participates in peer consultation and quality improvement activities.
  • Supervises physical therapy assistants and, when applicable, home health aides in accordance with organizational policy and state regulations, including required supervisory visits and evaluations.
  • Performs other job-related duties as assigned.
