Requirements

• Bachelor’s degree or higher from an accredited Physical Therapy program
• Current Physical Therapy license in the state of Arizona
• Minimum of 1 year of skilled home health experience
• Strong clinical, communication, and organizational skills
• Passion for patient care and improving lives at home

Responsibilities

• Partner with patients to restore mobility, function, and independence
• Develop and implement individualized, evidence-based care plans
• Track patient progress and adjust treatments as needed
• Encourage and motivate patients throughout their recovery
• Focus on safety, strength, flexibility, and long-term wellness
