About The Position

Physical Therapist – Skilled Nursing Facility Location: Orange, NJ A skilled nursing facility in Orange, NJ is seeking a licensed Physical Therapist to provide high-quality rehabilitative care to short-term rehab and long-term care residents. This role is ideal for a Physical Therapist with skilled nursing or post-acute experience who is comfortable working as part of an interdisciplinary care team.

Requirements

Licensed Physical Therapist (PT) in the state of New Jersey
1–2+ years of experience in a skilled nursing facility, long-term care, or post-acute rehab setting preferred
Strong knowledge of skilled documentation and Medicare Part A / Part B guidelines
Ability to manage a productive caseload in a fast-paced environment
Excellent communication and teamwork skills

Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans
Provide therapy services to short-term rehab and long-term care residents
Collaborate with nursing, therapy, and medical staff to support resident goals
Monitor patient progress and adjust treatment plans as needed
Document evaluations, progress notes, and discharge summaries in compliance with regulations
Participate in care plan meetings and interdisciplinary rounds
Support safe discharge planning and functional goal achievement
Ensure therapy services meet Medicare, Medicaid, and facility documentation standards
