Requirements

  • Current, unrestricted Physical Therapist (PT) License in the state of California.
  • Strong clinical reasoning and accurate documentation skills.
  • The ability to motivate residents and build positive relationships with families and colleagues.
  • A commitment to evidence-based practice and a passion for helping others regain their strength and independence.

Responsibilities

  • Evaluate residents and design evidence-based treatment plans, ensuring all services are approved by referring physicians.
  • Deliver high-quality, hands-on therapy in accordance with the established plan of care and the California state practice act.
  • Oversee Physical Therapist Assistants (PTAs) and provide guidance to Restorative Aides to ensure consistent care.
  • Maintain accurate evaluations, daily notes, progress reports, and recertification documentation in a timely manner.
  • Work closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.
  • Educate residents, families, and staff on safe mobility techniques and home-exercise programs to support successful transitions.
  • Recommend and coordinate necessary durable medical equipment (DME) such as walkers, braces, or grab bars.
