Requirements

Current, unrestricted Physical Therapist (PT) License in the state of California.
Strong clinical reasoning and accurate documentation skills.
The ability to motivate residents and build positive relationships with families and colleagues.
A commitment to evidence-based practice and a passion for helping others regain their strength and independence.

Responsibilities

Evaluate residents and design evidence-based treatment plans, ensuring all services are approved by referring physicians.
Deliver high-quality, hands-on therapy in accordance with the established plan of care and the California state practice act.
Oversee Physical Therapist Assistants (PTAs) and provide guidance to Restorative Aides to ensure consistent care.
Maintain accurate evaluations, daily notes, progress reports, and recertification documentation in a timely manner.
Work closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.
Educate residents, families, and staff on safe mobility techniques and home-exercise programs to support successful transitions.
Recommend and coordinate necessary durable medical equipment (DME) such as walkers, braces, or grab bars.
