Requirements

  • Successful completion of an accredited physical therapy education program
  • Current Oklahoma PT license in good standing
  • At least 1 year of physical therapy experience
  • CPR certification (or ability to obtain within 60 days)
  • Ability to meet the physical demands of the job

Responsibilities

  • Conducting initial and ongoing assessments to evaluate patient function
  • Collaborating with physicians to develop or revise individualized plans of care
  • Preparing clinical and progress notes in accordance with regulatory standards
  • Providing therapeutic treatments tailored to each patient's needs
  • Consulting with families and agency staff to support patient goals
  • Continuously evaluating treatment effectiveness and updating plans as needed
  • Coordinating services with other home health team members
  • Supervising physical therapy assistants and home health aides when appropriate
  • Assessing equipment needs to ensure optimal patient outcomes
  • Following CMS documentation and billing requirements, including functional reassessments
  • Practicing infection control and universal precautions
  • Reporting suspected abuse according to state and federal guidelines
  • Assisting with new‑hire orientation and participating in training activities
