Requirements
- Graduate of an accredited Physical Therapy program
- Active NY State Physical Therapy license
- Strong interest in working with geriatric, subacute, and adult patient populations
- Leadership qualities and ability to direct support staff when needed
- Graduation from an approved school of physical therapy required.
- State license to practice physical therapy required.

Responsibilities
- Initiate and implement personalized physical therapy programs based on physician orders
- Maintain detailed evaluations, progress notes, and discharge summaries
- Collaborate with nursing and rehab staff to enhance residents' functional outcomes
- Contribute to interdisciplinary care planning meetings
- Maintain treatment documentation, track patient statistics, and request necessary supplies
- Participate in or lead staff training and education
- Provide leadership support in the absence of the Director of Physical Therapy
