Requirements
- Active California Physical Therapist license (in good standing)
- Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
- Experience in skilled nursing or post-acute setting preferred
- Strong clinical, communication, and documentation skills
- Ability to lead, mentor, and collaborate effectively
- Comfortable with EMR systems such as Casamba and PointClickCare
Nice To Haves
- Experience in skilled nursing or post-acute setting preferred

Responsibilities
- Evaluate patients within 24 hours (or 1 business day) of referral
- Develop and implement individualized treatment plans
- Provide hands-on therapy in accordance with physician orders
- Supervise PTAs and support staff in patient care activities
- Collaborate with interdisciplinary team on patient progress and outcomes
- Participate in care conferences, discharge planning, and rehab meetings
- Train staff and caregivers on maintenance and restorative programs
- Ensure accurate, timely documentation per state and regulatory guidelines
- Assist with ordering adaptive and durable medical equipment
- Maintain therapy equipment and report any issues
