About The Position
Right at Home is a growing, locally owned home health company based in Cedar Rapids. We currently provide skilled nursing, companion, therapies and personal cares for a wide variety of clients in Eastern Iowa. We proudly provide these services to veterans, and we are in need of a part-time Physical Therapist to expand the level of skilled care available to them.
Responsibilities
- Provide high quality evaluation and treatment of neuromusculoskeletal disorders
- Develop personalized care plans to improve mobility, strength, and overall function of patients in the home setting, involving patient in setting goals.
- Communicate and collaborate with associated medical professionals to achieve patient goals
- Clear, concise documentation in our Clear Care software system
- Regular communication with physicians, nurses, case managers within Veterans Affairs as needed
- Ability to work independently and autonomously, managing one’s own schedule
- Supervise home health aides involved in patient care
