About The Position

Therapy Partner Solutions (TPS), therapist-owned and operated since 2016, is dedicated to providing exceptional therapy services. As the provider of home health therapy services for Community Home Care, TPS is part of a strong, multidisciplinary team that includes physical therapists, occupational therapists, speech therapists, nurses, physicians, and other healthcare professionals, delivering collaborative, coordinated, patient-centered care. The company understands the unique challenges of the home healthcare setting and supports its clinicians every step of the way. TPS invests in its people, offering opportunities to participate in its Clinical Excellence Academy, which includes mentorship, advanced training and certifications, virtual educational series, and OCS prep collaborative. The role involves working with a diverse and engaging caseload, supported by thoughtful scheduling, defined territories to limit drive time, clinical leadership by therapists, individualized support for professional growth, and collaboration with Community Home Care colleagues for a comprehensive, team-based approach to care. Requirements

  • Graduate (current or pending) from an accredited physical therapy program
  • Active or pending state physical therapy license
  • Comfort working independently in the home setting
