About The Position

We are seeking skilled Physical Therapists to join our actively growing Home Health Team with territories in St. Louis City and county, that will cover our Northern region traveling from Marconi north to O'Fallon, St. Louis County, and North County. Cooperative Home Care is a locally owned and operated home care company with a referral-heavy Home Health division. We are looking for physical therapists to cover the clients in a set territory. Company car and gas card are provided!

Requirements

  • Proven experience as a Physical Therapist
  • Valid license to practice physical therapy in the state of Missouri
  • Strong knowledge of anatomy, physiology, and rehabilitation techniques
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment

Responsibilities

  • Review and evaluate physician's or podiatrist's referral and patient's medical record to determine physical therapy required.
  • Perform patient tests, measurements, and evaluations, such as range-of-motion and manual muscle tests, gait and functional analyses, and body parts measurements, and record and evaluate findings to aid in establishing or revising specifics of treatment programs.
  • Administer manual therapeutic exercises to improve or maintain muscle function, applying precise amounts of manual force and guiding patient's body parts through selective patterns and degrees of movement.
  • Observe, record, and report to the physician or podiatrist the patient's treatment, response and progress.
  • Instruct patient and family in total physical therapy program.
  • Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.
  • Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
  • Inform patients when diagnosis reveals findings outside physical therapy and refer to appropriate practitioners.
  • Provide educational information about physical therapy or physical therapists, injury prevention, ergonomics, or ways to promote health.
