About The Position
FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Elizabethtown, KY to provide exceptional one-on-one care to older adults. This role involves delivering care in private residences and Senior Living Communities through the Geriatric House Calls™ model, focusing on improving mobility, independence, and quality of life. Clinicians will have autonomy and flexibility to create and manage their own treatment schedules and build strong patient relationships. The opportunity also allows for the incorporation of telehealth and virtual care to enhance flexibility and continuity of care. FOX Rehabilitation is a clinician-operated practice committed to meaningful relationships, evidence-based care, and abolishing ageism, supporting clinicians with leaders who understand clinical work.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
