About The Position

Jaffe Sports Medicine, a PartnerCare affiliated company, is seeking a full-time Physical Therapist to join our growing and established practice in North Naples, Florida. Our practice focuses on taking care of the patient with a comprehensive plan, delivering high customer service, keeping patients safe and well-informed and reducing patients' pain as much as possible. Our mission is to deliver comprehensive care to help musculoskeletal and pain management patients live their best lives possible. Our team of physicians, nurses, anesthesiologists, and staff are dedicated to ensuring that patients can improve their quality of life with pain relief that involves the use of cutting-edge technology. The Physical Therapist is responsible for delivering comprehensive rehabilitative care aimed at restoring function, improving mobility, relieving pain, and preventing or minimizing permanent physical disabilities in patients affected by injury or disease. This role also promotes overall fitness and health through individualized treatment plans and patient education.

Requirements

  • Masters / Doctoral Degree in Physical Therapy from an accredited university.
  • Current active and unrestricted (without sanctions) State License.
  • Basic Life Support/Advanced Cardiac Life Support certification.
  • Associates Degree in Physical Therapy from an accredited university or technical institution
  • Current physical therapist license
  • At least three years of experience as a physical therapist
  • Knowledge of legal, medical, and accreditation compliance
  • Alertness and careful attention to detail is required to avoid injury. Must be able to deal with stressful and/or complex situations
  • Current physical therapist license
  • Must be able to physically work with the patient to ensure exercises are performed correctly

Responsibilities

  • Evaluate patients, perform initial and ongoing assessments, and develop individualized treatment plans
  • Deliver therapeutic interventions including manual therapy, exercise programs, functional training, and use of assistive devices
  • Educate patients and families on treatment plans, home exercises, and injury prevention
  • Monitor patient progress and adjust care plans to maximize outcomes
  • Accurately document evaluations, treatments, and progress in the EMR
  • Collaborate with physicians and interdisciplinary team members
  • Maintain compliance with HIPAA, OSHA, infection control, and organizational policies
  • Promote a safe, clean, and patient-centered clinical environment
  • Provide excellent patient service and address patient concerns professionally
