About The Position
We are looking for an experienced and motivated Physical Therapist to join our growing team in Williamsville, NY. Physical therapist position details: We are looking for a per diem (as needed) physical therapist to provide coverage for vacations, leave time, and sick time. Applicants would need availability Monday – Friday, day shift.
Requirements
- Current and valid NYS license to practice as a physical therapist is required.
- Must have the ability to work independently and as part of a team.
- Must be able to communicate well verbally and in writing.
- Proficiency with electronic medical record (EMR) system is required.
Nice To Haves
- Experience in subacute rehab or outpatient geriatric settings is preferred.
Responsibilities
- Performing skilled therapy assessments and evaluations.
- Developing and implementing individualized treatment plans.
- Administering a range of physical therapy techniques and modalities.
- Monitoring patient progress and adjusting treatment plans as needed.
- Maintaining accurate and timely documentation in accordance with progression and regulatory standards.
- Collaborating with interdisciplinary team to ensure coordinated care.
