Job Description: Physical Therapist – Home Health Coverage Area: In and around Worcester, MA Sign-On Bonus: $2,500 At Elara Caring , we believe the best place for care is where patients live. As a Physical Therapist , you’ll play a vital role in helping patients regain independence and improve quality of life—all while working in a collaborative, supportive environment.

Requirements

• Graduate of an APTA-approved Physical Therapy program
• Current, unrestricted PT license in Massachusetts
• Minimum 1 year of experience in a healthcare setting
• Ability to travel within Worcester and surrounding areas (50% travel)
• Reliable vehicle, valid driver’s license, and current auto insurance
• Ability to lift 50–100 lbs and perform full range of body motion

Responsibilities

• Provide direct patient care, evaluate functional needs, and design individualized care plans
• Collaborate with other healthcare professionals to ensure optimal outcomes
• Document evaluations, treatment goals, and progress
• Act as a resource and promote physical therapy in the community
• Work in a physically demanding, high-energy environment
